Association of Monocyte Chemoattractant Protein-1 in gingival crevicular fluid and saliva of chronic periodontitis subjects with cardiovascular diseases – A case control study

Abstract
Objective: To determine the association of elevated levels of Monocyte Chemo attractant Protein-1 (MCP-1) in saliva and gingival crevicular fluid of severely affected chronic periodontitis patients with well-established progression and acute precipitation of cardiovascular diseases.
Methodology: This case-control study included 100 subject between the ages of 35-68 years. Cases with severe periodontitis along with horizontal alveolar bone loss (grade 4) having cardiac disease were selected and individuals with periodontitis without cardiac diseases were opted as controls. A structured questionnaire was used to collect detailed demographics, cardiovascular, medical, and family history of the patients. Grading of alveolar bone loss, periodontal and gingival status, probing depth, gingival index, plaque index and clinical attachment level were recorded. Gingival crevicular fluid and salivary sample were extruded for the biomarker and quantified in a solid phase, sandwich technique using Elisa kit.
Result: Periodontal assessment in cases revealed that 4% had moderate and 96% had severe form of periodontitis with 24% cases had grade-3 and 76% cases had grade-4 horizontal alveolar bone loss. The chi square test showed significant differences in the proportions (p<0.05). Mean comparison of MCP-1 between cases and control groups showed that in control group mean MCP-1 was 286.5±54.3, whereas in cases the mean MCP-1 was 464.4±175.2). Independent sample t-test showed a significant mean difference for MCP-1 between cases and controls (p<0.05).
Conclusion: The significant positive results observed in cases with severe form of periodontitis suggest that the inflammatory mediator, MCP-1 is responsible for progression of acute precipitation of cardiovascular diseases.
