Tracheocele presenting as a case of persistent difficult ventilation and subcutaneous emphysema post tracheostomy

Abstract
Tracheocele or tracheal diverticulum is rare, and seldom reported and majority are asymptomatic. We report a case of a tracheocele detected after persistent difficult ventilation post tracheostomy, with tube changing difficulty and persistent subcutaneous emphysema. Flexible endoscopy was performed and confirmed a blind ending pouch at the posterior tracheal wall. The tip of tracheostomy tube was repositioned beyond the pouch and emphysema resolved.
