Efficacy of topical DMSO in macular amyloidosis

Abstract
Objective: To evaluate the efficacy of 50% topical Dimethlsulfoxide (DMSO) in Macular Amyloidosis.
Methodology: This cross sectional, interventional study was conducted in dermatology department, Abbasi Shaheed Hospital, Karachi, Pakistan from October 2011 to April 2012 . A total of 50 patients of ≥16 years of age, clinically diagnosed with Macular Amyloidosis were included via purposive sampling. They were advised to apply thin layer of 50% DMSO on affected area at night. At presentation, patients were assessed for presence of pigmentation and pruritis was graded subjectively by patient on 10 cm. visual analogue scale (VAS) as absent (0), mild (1-3), moderate (4-7) and severe (˃7). Successful response was recorded at end of 3rd month of therapy in terms of complete resolution of pigmentation and zero or mild pruritis.
Results: Among 50 patients, moderate pruritis was present in 21 (42%) patients while severe pruritis in 24 (58%). Pigmentation was present in all 50 (100%) patients. At end of 3 months treatment, pruritis resolved to absent (0) in 49(98%) patients and only 1 patient (2%) had moderate pruritis. Whereas, pigmentation was resolved in only 4 (8%) patients.
Conclusion: By DMSO application, satisfactory response was observed in itching. We hope to observe response in pigmentation also on long term treatment
