Abstract
Objective: To assess the evidence for the OAS1 rs10774671 genetic variant association with COVID-19 severity and associated risk factors in the Pakistani population.
Methodology: In this analytical cross-sectional study, 200 PCR-confirmed COVID-19 patients were genotyped by TaqMan real-time PCR, with 6% of samples validated by Sanger sequencing. Associations were assessed using Chi-square tests and multivariable logistic regression adjusted for age, sex, BMI, and comorbidities.
Results: Participants had AA (75), AG (71), and GG (54) genotypes. The AA genotype demonstrated nearly a 3-fold higher likelihood of developing severe COVID-19 compared to the mild/moderate group (OR=2.99; 95% CI=1.30–6.86; p=0.0073). Among 143 active COVID-19 patients (71 severe and 72 mild/moderate), higher BMI (aOR=1.20 per kg/m², 95% CI: 1.1–1.32; p<0.001), comorbidity (aOR=9.22, 95% CI: 1.88–45.18; p<0.0006), were independently associated with severe COVID-19, whereas age and sex were not.
Conclusion: OAS1 rs10774671 was associated with COVID-19 severity, with the AA more prevalent among severe cases.

Key words: Genetic risk factors, single nucleotide polymorphism, OAS1, COVID-19.
